More an art than a science, treating diabetes requires experience and careful consideration to create a care plan that’s as unique as the patient.
At Oak Street Medical, we take the time to get to know our patients, knowing that family, work, diet, exercise and emotional well-being all play an important role. By considering the entire person, we’re better able to optimize your overall health.
Whether you have diabetes or are the parent of a child with diabetes, know that our compassionate and caring staff will work hard to find the right tools for you. Using the latest technology, and the most well-rounded integrated care, Oak Street Medical offers our patients the very best in diabetes care.
Our well-respected team is led by Dr. Kirk Jacobson, who has spent 30 years treating adults and children with diabetes. Three of Dr. Jacobson’s family members have diabetes, which means he understands what it’s like to live with the disease. “Living with someone who has diabetes is much different than reading about it in books,” he says.
Supported by a highly trained and experienced team of registered nurses and certified medical assistants, our diabetes providers are enthusiastic about helping people.
Convenient and reliable, our on-site lab tests and collection station means rapid results and treatment. Our on-site lab tests include: urinalysis, blood sugar, hemoglobin A1c, glucose tolerance and more.
New patients can normally be seen within one to two days. We offer same-day visits for current patients, regular exams and long-term care. Nurse appointments are also available for blood draws, injections, and routine monitoring.
